Windows NT, 2k, XP: Dial-up networking uses the phonebook file: rasphone.pbk which should be in the Windows\system32\RAS folder. Make sure the file is in this folder. (Use FIND files). If the file is missing, remove and re-install networking components including dial-up networking.
This error should not occur with Windows 9x/Me.
Additional feedback:
I've just installed BT broadband and have XP (SP 1) and tried re-install networking components. This didn't work and it didn't put the rasphone.pbk into the folder.
However it did put me on the right trail. I manually copied rasphone.pbk from C:\Documents and Settings\All Users\Application Data\Microsoft\Network\Connections\
into
Windows\system32\RAS folder
